Rates and Insurance
Kim is licensed in Oregon and Washington and is in-network with Regence BlueCross BlueShield of Oregon, Moda Health, Aetna, Cigna, and CareOregon plans.
Caleb is licensed in Oregon, Washington, and California. He is currently in the process of insurance credentialing and is accepting cash-pay patients at this time.
You can find our cash pay rates on the Team Page. Sliding scale options may be available on an individual basis. We can also provide superbills for out-of-network reimbursement, depending on your insurance plan.
We do our best to provide cost estimates based on the insurance information available to us, including your current deductible status. However, insurance information is not always fully up to date, and your final cost may vary slightly. Ultimately, it is your responsibility to understand your insurance plan, including your coverage, deductible, copays, and out-of-pocket costs.

We provide comprehensive clinical ADHD assessments for adults. The evaluation typically takes two to three separate, hour-long appointments and includes self-assessment questionnaires completed ahead of time to help guide the process. Our goal is not only to determine whether ADHD is the right diagnosis, but also to carefully assess for co-occurring conditions that may overlap with or mimic ADHD. This ensures an accurate, thorough, and individualized understanding of your symptoms and needs.

We offer collaborative medication management with a focus on safety, clarity, and individualized care. Before prescribing any medication, we may request previous medical or psychiatric records to ensure an accurate understanding of your history and to make the most informed treatment decisions. Our approach emphasizes using the lowest effective dose and integrating holistic strategies to support your overall wellbeing.
Follow-up appointments are typically spaced between 2–12 weeks depending on your treatment plan and stability. At minimum, we require a check-in every three months to ensure ongoing safety, effectiveness, and continuity of care.

While psychotherapy services are not currently offered at Tidal Health NW, we value the essential role therapy plays in mental health treatment. If you are not already engaged in therapy services, we are happy to help connect you with trusted therapists, counselors, or community resources, and we strongly encourage a collaborative approach. With your permission, we coordinate care with your therapist or other providers to support a well-rounded, integrative treatment plan.

Accessibility
Our office is on the second floor of a building without wheelchair access. If you need accommodations due to mobility challenges, please contact our office so we can work with you. We have multiple options including telehealth appointments, or an alternate location for a single appointment.
Cancellation Policy
Cancellations must be made at least 48 hours prior to your appointment to avoid a cancellation fee. Typically, a patient must be seen a minimum of every three (3) months or more frequently, as indicated by the treatment plan, for continuation of the provider-patient relationship.
Coverage Changes
We understand that insurance changes can be stressful, especially when it affects access to mental health care and medications. If you lose coverage or your insurance changes, we will do our best to help provide bridge coverage for necessary medications and support a smooth transition of care.
This may include short-term medication refills when clinically appropriate, helping you understand your options, and assisting you in getting established with a new provider or care team if needed. Our goal is to help prevent unnecessary gaps in treatment and support continuity of care whenever possible.
