Rates and Insurance

Kim is licensed in Oregon and Washington and is in-network with Regence BlueCross BlueShield of Oregon, Moda Health, Aetna, Cigna, and CareOregon plans.

Caleb is licensed in Oregon, Washington, and California. He is currently in the process of insurance credentialing and is accepting cash-pay patients at this time.

You can find our cash pay rates on the Team Page. Sliding scale options may be available on an individual basis. We can also provide superbills for out-of-network reimbursement, depending on your insurance plan.

We do our best to provide cost estimates based on the insurance information available to us, including your current deductible status. However, insurance information is not always fully up to date, and your final cost may vary slightly. Ultimately, it is your responsibility to understand your insurance plan, including your coverage, deductible, copays, and out-of-pocket costs.

Accessibility

Our office is on the second floor of a building without wheelchair access. If you need accommodations due to mobility challenges, please contact our office so we can work with you. We have multiple options including telehealth appointments, or an alternate location for a single appointment.

Cancellation Policy

Cancellations must be made at least 48 hours prior to your appointment to avoid a cancellation fee. Typically, a patient must be seen a minimum of every three (3) months or more frequently, as indicated by the treatment plan, for continuation of the provider-patient relationship.

Coverage Changes

We understand that insurance changes can be stressful, especially when it affects access to mental health care and medications. If you lose coverage or your insurance changes, we will do our best to help provide bridge coverage for necessary medications and support a smooth transition of care.

This may include short-term medication refills when clinically appropriate, helping you understand your options, and assisting you in getting established with a new provider or care team if needed. Our goal is to help prevent unnecessary gaps in treatment and support continuity of care whenever possible.
